You are not required to select whether you are signing up for a physical therapy or wellness visit. Instead, you are simply scheduling time with Kristie and will be able to discuss details at your appointment.

After scheduling an initial assessment, you should receive an email with intake paperwork. I have streamlined the paperwork to simply include basic information and the necessary consent forms. This should take 5 minutes or less to complete.
